What to Expect During Your First Acupuncture Session

Considering acupuncture for the first time? Here's a complete guide to what happens during your first session — from assessment to needle insertion to aftercare.

By Brian Abbott, R.Ac

If you've never had acupuncture before, it's natural to feel a mix of curiosity and apprehension. You're not alone — many of my patients at The Fixx arrive for their first session not quite knowing what to expect. My goal is always to make that first experience comfortable, informative, and effective.

Acupuncture has been practiced for thousands of years, and modern research continues to validate its effectiveness for a wide range of conditions. But understanding what actually happens during a session can help you approach it with confidence rather than uncertainty.

Before Your Appointment

A little preparation goes a long way toward a positive experience.

What to Wear

Wear loose, comfortable clothing. Acupuncture points are located all over the body, but many of the most commonly used points are on the arms (below the elbows) and legs (below the knees). Loose clothing allows me to access these areas while keeping you comfortable and covered.

Eat Before Your Session

Don't arrive on an empty stomach. Having a light meal or snack 1–2 hours before your appointment helps stabilize your blood sugar and reduces the chance of feeling lightheaded during or after treatment.

Hydrate

Drink plenty of water before your appointment. Good hydration supports your body's response to treatment.

Avoid Caffeine and Alcohol

Try to avoid caffeine for a few hours before your session, as it can make it harder to relax. Avoid alcohol entirely on the day of your treatment.

The Assessment

Your first session begins with a thorough assessment. This is a crucial part of the process — acupuncture is not a one-size-fits-all treatment, and I need to understand your unique situation before designing your treatment plan.

What I'll Ask

  • Your primary complaint and health history
  • When your symptoms started and what affects them
  • Your sleep, digestion, energy levels, and stress
  • Your lifestyle, work, and activity levels
  • Any medications or supplements you're taking

What I'll Observe

In Traditional Chinese Medicine, assessment includes observing signs that reveal information about your internal balance. I may look at your tongue (its colour, coating, and shape provide diagnostic information), feel your pulse (the quality of your pulse tells me about the state of different organ systems), and palpate acupuncture points and areas of tension.

This assessment takes time — usually 20–30 minutes of your first session. It's time well spent, as it allows me to create a truly personalized treatment plan.

The Treatment

Positioning

You'll lie comfortably on the treatment table — usually face up, though some points may require you to lie on your side or stomach. I'll make sure you're warm and comfortable with blankets and pillows.

Needle Insertion

Acupuncture needles are extremely thin — much thinner than the needles used for blood tests or injections. They're about the width of a human hair, and most patients feel little to nothing when they're inserted.

I'll insert needles at specific points based on your assessment. The number of needles varies — typically anywhere from 8 to 20, depending on your condition and treatment plan.

What You'll Feel

When a needle is inserted, you may feel:

  • Nothing at all — many insertions are completely painless
  • A slight pinch or tap — brief and mild
  • A dull ache, heaviness, or warmth — this is the "de qi" sensation, a sign that the acupuncture point is being activated

If a needle feels uncomfortable or sharp, tell me immediately. Your comfort is always the priority.

Resting with the Needles

Once the needles are in place, you'll rest for 20–30 minutes. This is your time to relax. Many patients find this deeply peaceful — some even fall asleep. The needles work while you rest, and the stillness allows your body to respond to the treatment.

After Your Session

Immediate Effects

Most patients feel deeply relaxed after their first session. Some feel energized, others feel sleepy. Both are normal. You may notice immediate changes in your symptoms, or the effects may develop over the next 24–48 hours.

Self-Care After Treatment

  • Drink water: Stay hydrated to support your body's processing of the treatment.
  • Rest: Avoid strenuous activity for the rest of the day if possible.
  • Avoid alcohol: Give your body time to integrate the treatment.
  • Notice changes: Pay attention to how you feel over the next few days.

Treatment Frequency

For most conditions, I recommend an initial course of 4–6 weekly sessions. Acupuncture is cumulative — each treatment builds on the last. After the initial course, we'll reassess and adjust the frequency based on your progress.

Common Questions

"Does it hurt?"

This is the most common question I hear. The honest answer is: very little, if at all. The needles are hair-thin and insertion is gentle. If you've been avoiding acupuncture because of needle fear, I encourage you to try just one session — you'll likely be surprised at how comfortable it is.

"Is it safe?"

Acupuncture is extremely safe when performed by a qualified practitioner. I use only sterile, single-use, disposable needles. Side effects are minimal — occasional minor bruising at a needle site is the most common.

Your first acupuncture session is the beginning of a journey toward better health. I'm here to guide you through it with expertise, care, and genuine commitment to your wellbeing.
